This woman desired reshaping of her breasts. Often times, as a consequence of pregnancy (especially breast feeding) and natural aging process, a woman's breasts undergo changes leading to a "saggy" appearance. During the consultation, several key points were discussed: The most important being the understanding of the dynamic relationship between skin envelope and breast volume. Ideally, there is a "perfect" fit – i.e. the amount of skin surrounding the breast tissue is just right. However, as most woman know all too well, with pregnancy and breast feeding (and gravity), skin envelope tends to increase while breast volume deflates leading to "saggy" or "droopy" breasts with upper pole deflation. With this in mind, we can either do one of three things: 1) increase the breast volume by adding breast implants to "fill up" the excess skin envelope – realizing that the breast size may be far larger than what the patient desires, 2) shrink the skin envelope with nipple repositioning without breast implants – realizing that additional scarring will be involved, or 3) increase the breast volume moderately with some degree of skin reduction (combined breast lift (mastopexy) augmentation). Upon discussing the options, patient decided that she was content with her breast size, and therefore, did not want any additional volume (breast implants); furthermore, she was accepting of the anticipated scar that would be involved with full breast lift (mastopexy). She uneventfully underwent a breast lift (mastopexy) procedure and was very pleased with her new "lifted" breast shape. The photos shown were taken at 6 weeks postoperatively to illustrate how "red" they can appear early on, but as with most scars, they will mature and fade into less noticeable scars over time.
